Meet
Jacob Jacob's
Team | Jacob's team is made up of staff from the
Lawrence Public Schools, Raintree Montessori and his family. Jacob's
team includes: a physical therapist, a speech language pathologist,
an inclusion facilitator (graduate student from the university), an
occupational therapist, an early childhood special education teacher,
his mom, and the assistant classroom teacher. Jacob's team is unique
in the fact that his classroom teacher is a certified early childhood
special education teacher. She is employed by Raintree.
The core team which includes those who work with him on a daily basis meets every other week to plan activities and report progress. The team uses a 3 ring notebook to help the professionals and parents communicate. When a service provider comes into the classroom and works with Jacob she makes a note in the communication book so that other service providers and Jacob's mom can see what he has been doing. The larger team which includes administrative personnel and consultative professionals such as the school nurse and the school social worker meets monthly. |
