Transition from the Early Intervention Program

Children and families who received early intervention services (0-3 years) through a Part C local program should initiate discussions about transition when the child turns three early. In fact, many authors recommend starting the transition "out" process when the child "enters" the early intervention program. At a minimum, the discussion should begin six to twelve months before the child's third birthday. Many resources to assist families and programs with transition are available through state and local interagency coordinating councils.
