Meet Transition Facilitators Working in Inclusive Settings

This facilitator provides support and coordination of transitions of each child from school to school. This role focuses entirely on support for the students. In other words, this specialist is a counselor, a manager of student records, and an evaluator who assesses the progress of students, as well as the effectiveness of the transition coordination services.
